Participant Waiver & Release of Liability

I understand that participating in a 5km run involves physical activity and carries inherent risks, including the possibility of injury, illness, permanent disability, or, in rare cases, death.

By submitting this form, I confirm that I am voluntarily participating in this event, am in good physical health, and am medically fit to take part. I further confirm that I have not been advised by a qualified medical professional against participating and that there are no medical conditions that would make my participation unsafe.

I knowingly and voluntarily accept full responsibility for all risks associated with my participation, whether known or unknown, including any injury, illness, property damage, loss, or other consequences that may arise before, during, or after the event.

In consideration of my participation in this event, I agree to the following on behalf of myself, my heirs, executors, administrators, successors, and assigns:

A. Waiver & Release

I hereby waive, release, and forever discharge Spa Ceylon, Port City Colombo, Asiri Health, and their respective representatives, directors, officers, employees, volunteers, partners, sponsors, and event organizers from any and all liability, claims, demands, or causes of action arising out of or relating to my participation in this event, including those resulting from negligence, to the fullest extent permitted by law.

B. Indemnity

I agree to indemnify, defend, and hold harmless the above-mentioned parties from any claims, liabilities, damages, costs, or expenses arising out of or relating to my participation in this event.

C. Rules & Third-Party Disclaimer

I confirm that I have read, understood, and agree to comply with all event rules, instructions, and safety guidelines. I acknowledge that Spa Ceylon, Port City Colombo, Asiri Health, their representatives, directors, officers, employees, volunteers, partners, sponsors, and event organizers are not responsible for the acts, omissions, errors, or failures of any third party involved in conducting any part of the event.

D. Media Consent

I also grant Spa Ceylon and its authorized representatives permission to capture and use photographs, videos, audio recordings, or any other media of me taken during the event for promotional, marketing, editorial, or other lawful purposes without compensation.

By submitting this form, I confirm that I have carefully read, understood, and voluntarily agree to the terms and conditions of this Waiver & Release of Liability.
